House Coordinator

Job Description

General: The House Coordinator is responsible for maintaining and assuring a quality of life within an assigned home that reflects the values, goals, and objectives outlined in the Friendship Ark Homes Mission Statement. To the extent possible, the House Coordinator will actively participate in the daily life of the home’s core members. The House Coordinator will provide core member support and foster participation in home and community activities, with a focus on the development of independent and interdependent living skills. The House Coordinator will hold periodic house meetings with the core members to encourage the participation of daily living in a family style home.

Pay Grade: 24

Reports To: Supervisor

Supervises: All employees and volunteers associated with the designated home.

Status: Non-Exempt

Responsibilities:

General Duties: Performs all duties as outlined in the Live-Out Assistant’s job description. In addition coordinates, oversees, and provides direct service in the following areas of activity:

1. Shopping

  • Overseas menu planning and grocery shopping.
  • Assures personal care item purchases for core members as needed.
  • Coordinates house supply purchases.

2. Maintenance and Cleaning

  • Coordinates maintenance of house with Supervisor's and Executive Director's advice.
  • Coordinates vehicle maintenance.
  • Reviews mileage logs.
  • Creates responsibility lists with and for core members.

3. Safety

  • Conducts safety drills as deemed appropriate for the home.
  • Performs Monthly Safety Checklist duties.
  • Reviews safety plans quarterly with core members.
  • Provides safety orientation for staff and volunteers at the home.
  • Performs quality assurance reviews for safety/cleanliness of equipment and property.

4. Appointments

  • Coordinates all core member appointments with guardians and RN.
  • Attends core member appointments when appropriate.
  • Participates in monthly House Coordinator, staff, and Leadership meetings.
  • Coordinates and participates in house guest events for core members.
  • Participates in core member meetings.
  • Reports appointment outcomes to RN, Supervisor, and direct care staff.

5. Documentation

  • Reviews Medication Administration Records: passed/signed/current. Follows-up with staff regarding med errors.
  • Reviews daily progress notes and verifies goal achievements, creating weekly Quality Assurance data.
  • Drafts and reviews core member's quarterly/annual progress reports, with Supervisor.

6. Finances

  • Assists core members with personal financial management.

7. Supervisory

  • Oversees the work of staff and volunteers. Provides individual core member training to all new incoming staff.

8. Other:

  • Verifies monthly forms and records are available.
  • Other duties as assigned.

Classification: This position requires tasks that routinely involve a potential for mucous membranes or skin contact with blood, body fluids, tissues or potential spills or splashes. Use of appropriate measures is required for every health care provider in this position.

Qualifications:

Age: Must be 18 years of age or older.

Education: Two years beyond High School or four years fulltime experience. Certification of Medication Manager (must meet requirement within 90 days of employment).

Experience: One year fulltime experience living with or working with adults or children with special needs. Supervisory experience preferred.

Driver’s License: Must have a valid license and qualify as an insurable driver.

Physical Demands: This person will be in good health, free from communicable diseases, and able to lift and carry up to 50 pounds.

Personal Qualities: Has a demonstrated commitment to meeting physical and spiritual needs of persons with disabilities. Discrete and confidential in handling private information about self and others. Demonstrates a willingness to work as a team, is well organized, exemplifies good leadership skills, is able to plan and supervise the work of others, and has strong oral and written communication skills. Able to work unsupervised, coordinating direct care with indirect duties. Must pass DCI and other background checks.
