Weekend Shift Coordinator

Job Description

General: The Weekend Shift Coordinator is responsible for maintaining and assuring a quality of life within an assigned home that reflects the values, goals, and objectives outlined in the Friendship Ark Homes Mission Statement. To the extent possible, the Weekend Shift Coordinator will actively participate in the daily life of the home's core members. The Weekend Shift Coordinator will provide core member support and foster participation in home and community activities, with a focus on the development of independent and interdependent living skills. The Weekend Shift Coordinator maintains the routine living environment of the home while providing overall responsibility of service areas, such as Medications, Core Member Activities, Safety, and Documentation. The Weekend Shift Coordinator's responsibilities fall within the time period of 6 p.m. Friday until 6 p.m. Sunday.

Pay Grade: 21

Reports To: Supervisor

Supervises: All employees and volunteers associated with the designated home throughout the weekend.

Status: Non-Exempt

Responsibilities:

General Duties: Performs all duties outlined in the Live-Out Assistant's role description. In addition coordinates, oversees and provides direct service in the following areas of activity:

Shopping

  • Coordinates menu planning and grocery shopping as needed for the weekend.

Maintenance and Cleaning

  • Maintains vehicle gasoline supply.
  • If needed maintenance is discovered for the house or a vehicle, a Work Order form will be completed and given to the House Coordinator.

Safety

  • Provides a safe environment for staff and volunteers at the home.
  • Provides emergency medical treatment as needed and reports outcomes to House Coordinators and Supervisors.

Medications

  • Reviews MARs for changes and/or upcoming meds for the weekend.
  • Follows up, gets signatures, documents details of Med Errors that occur during the weekend.

Documentation

  • Reviews daily Progress Notes and verifies goal achievements for services provided during the weekend.

Report

  • Completes a Shift Coordinator Report form that includes:
    • Shopping Activity
    • Maintenance Needs
    • Safety Issues
    • Med Errors
    • Documentation Needs

Supervisory: Oversees the work of staff and volunteers throughout the weekend at the designated home.

Other: Other duties as assigned.

Classification: This position requires tasks that routinely involve a potential for mucous membranes or skin contact with blood, body fluids, tissues or potential spills or splashes. Use of appropriate measures is required for every health care provider in this position.

Qualifications:

Age: Must be 18 years of age or older.

Education: Two years beyond High School or four years full time experience. Certification of Medication Manager, CPR, and First Aid preferred (must meet requirement within 90 days of employment).

Experience: One year full time, or two years part time experience living with or working with adults or children with special needs.

Driver's License: Must have a valid license and qualify as an insurable driver.

Physical Demands: This person will be in good health, free from communicable diseases, and able to lift and carry up to 50 pounds.

Personal Qualities: Has a demonstrated commitment to meeting physical and spiritual needs of persons with disabilities. Demonstrates a willingness to work as a team, is well organized, exemplifies good leadership skills, is able to plan and supervise the work of others, and has strong oral and written communications skills.
